I am trying to get a form created in Orders.html to submit back to a Active Serve Page, into a Access database.

I keep getting errors and not sure exactly what I am doing incorrectly. I am new at this and need this sorted out asap. Any help anyone can offer most appreciative. Perhaps someone wouldn't mind re-writting my asp page? I am also using a DSN with a order.inc file, and everything is set up correctly in the ODBC Data Source system, along with all files in the same directory.

Can anyone please offer suggestions as to why this isn't connecting to the database, to submit information from the form into the database table called "Orders".
